Tips for completing a mandate form
When filling out the mandate form, please follow these steps:
- Use black ink when signing the form.
- If you need to make any changes after printing, just use black ink and initial any updates.
- Don't shorten, abbreviate or change your business or trading name.
If you're adding or changing a signature, we'll need:
- Photo ID, like a passport or driving licence.
- Proof of address, such as a recent utility bill or bank statement.
